4. Did You Speak with a Admin Or Moderator? * -

5. What Server did this occur on? *  -

Please fill out all the information and send a private message to lKanyoNl and he will review the information and will send you a response once the investigation has been completed.

Once a private message has been completed fill out a section saying:

"Request Ban" and state wating ban approval.
